At MagnaCare, we understand your privacy concerns and we share your concerns regarding the confidentiality of your protected healthcare information:

• We extend our privacy principles to all of our business relationships. We expect our business associates and contracted vendors to honor and follow our privacy policies when they deal with your protected healthcare information and we hold them accountable for such adherence. Our business associates and contracted vendors may include entities that provide specialized services such as claim administration, out of area network access or pharmacy benefit management. We also hold our employees responsible for complying with our policies. Each MagnaCare employee is trained on the importance of understanding and following our privacy.

Information We Collect
We try to limit our collection of information to that which we require to properly perform our services and to fulfill our legal and regulatory requirements, such as the following:

• Name, address, telephone number and/or email address;
• Names, addresses, telephone numbers and/or email addresses of your spouse and dependents;
• Your social security number, age, gender, and marital status;
• Any information that we receive about you and your family from your applications or when we administer your policy, claim, or account;
• Any medical information required for management of your healthcare benefit and processing your claim.

Our Commitment to Confidentiality

• We apply our privacy policies to protected healthcare information, that is, information about you that could directly or indirectly identify you, whether in electronic or hard copy format, whether original or copied, whether free-standing or networked, and contained in any medical records.
• We use physical and technical security safeguards to protect your protected healthcare information from unauthorized disclosures. We limit access to, and use of, protected healthcare information to those individuals in our organization who must have such access in order to perform their responsibilities.
• We limit the release of your protected healthcare information other than as is minimally necessary for our administration, adjudication or other servicing of your claim or account, or other purposes permitted by law. We do not share protected healthcare information, except as permitted by law and we do not market such information in any event.
